Faith-Based Services
Four Faith-Based Resource Centers provide resources to individuals released from custody. Interested individuals must first register in person at one of the Reentry Resource Centers in San Jose or Gilroy to be connected to one of the centers below.

The four Faith-Based Resource Centers serve justice involved adults residing in Santa Clara County. They offer an open, safe, and welcoming environment and a wide range of services regardless of a client’s faith. The centers work collaboratively with other faith-based communities and other County and community partners to provide individuals with the necessary community supports for successful reintegration in the community. Clients can receive wrap-around services through case management, mentorship, one-touch service linkages, and other community resources.

Breakout Prison Outreach

A program assisting clients in navigating and accessing available resources throughout the adult reentry network and in the community. Once in-custody clients are identified, Breakout Prison Outreach (BPO) staff will conduct one-on-one visits with them to create a pre-release discharge plan addressing their basic needs upon release. BPO staff will provide post release service navigation support through case management and individualized care plans, weekly support groups and monthly pro-social activities.
San Jose State University Research Foundation
San Jose State University Research Foundation supports individuals prior to release or who have been recently released through service navigation case management. Intensive services will be concentrated during the critical first month of release with intensity tapering off as the clients stabilize. SJSU staff will assess each individual’s needs and assist in eliminating barriers to self-sufficiency through planning and service linkage/navigation. These barriers may include: housing, mental health and substance use, physical health, education, etc. Direct support services provided are transportation, assistance with job and housing search and paperwork.
Turn Behavioral Health Systems (Linked)
The Linked Program supports individuals exiting incarceration or who have been recently released. Staff will engage with identified clients 30 days prior to release to create a plan focusing first on their immediate needs (transportation, housing and appointments) with frequent contact post release to work on individualized case plans. The primary goal is to assist reentry clients in navigating available reentry and community-based resources including mental health/substance use, legal, education, employment, support groups and housing while working towards self sufficiency.
